docs: Consolidate project docs into Sphinx docs
    
    This commit does the following:
    
    * Converts contributing and maintainer guidelines from hidden Markdown
      files into existing Sphinx documentation
    * Reorganize docs index page to add multiple categories
    * Update README to point to new URLs for contributing guidelines
    
    I tested this locally in my environment and confirmed the docs render as
    expected.
