The objective of this ticket is investigate the common topics used across teams for email introduction and propose a general template for the on boarding process.
A minimum set of data could be:
Name FAS Account Country (Useful for stats) IRC nickname Participation in another FLOSS projects Interests Why are you interested in joining the team? etc
Further this can be implemented using functionality in Mailman/Hyperkitty.

@amsharma I don't understand what's the difference between #95 and #96
@jonatoni #96 is the response mail that should go from D&I team to the Introduction mail of any new member. #95 is for the new members to guide them what should be included in the introduction mail. HTH.
I am +1 to details requested above, as long as they are optional. I don't think it is a requirement for new members to mention all of these in their introductory email.
I think the best page to put this would be our team docs. Additionally, I think it always helps to give supporting information on why we are requesting these details. Here is my proposed template -
Here are some details you should mention in your introduction to the Diversity and Inclusion Team :
Preferred Name (Required) FAS Account (Required) IRC nickname (Required) Why are you interested in joining the Fedora D&I team? (Required)
The following details are optional but can be helpful for D&I team to suggest newcomer tasks tailored to your interests
Participation in Fedora community and/or other FLOSS projects (Optional) Country (Optional) Background and Interest areas (Optional)

For context, the CommOps ticket linked in the original comment was meant to provide an email template for all sub-projects / SIGs, not just CommOps.
We already have an introduction template in our documentation, which is where we point people for joining the team. A few of our old wiki pages redirect to this page:
