Survey Request Template

This template can be used by anyone who wants to request a survey on Fedora Project's instance LimeSurvey (should be related to Fedora project use cases).
The requests are monitored and acted upon by mindshare. Vote of the FCAIC is compulsory for this ticket.
Once approved, please share a survey structure file (.lss) exported from the your personal LimeSurvey account (to know how: visit https://manual.limesurvey.org/Display/Export_survey)

The questions look appropriate, so +1 to the survey.

My suggestion would be to reword them so you can get appraising and constructive feedback from everyone. So I would remove the conditions like "if you picked yes/no, " and instead just ask:
- Which communication channels have been most useful for you to learn about Fedora CoreOS (Blog, ...)?
- What would you like us to do to improve our communication?

Also instead of "If you would like to share any other feedback with us, use this field" maybe use "What else should we know?" or "What else would you like to tell us?".

I am +1 to the survey and will get it in limesurvey as soon as @riecatnor gives her +1 :)

Thanks for taking a look at it :smile: . I am far from being a survey specialist but I think the goal of having a Yes/No answer first was to have a low barrier of engagement then the followup questions are for people that wish to add more context to their answer.
I feel that I am more likely to select Yes/No in a survey than typing a answer to a question. Having a simple Yes/No answer first also gives us a good way to visualize the results (ie 51% answered Yes, 49% answered No ...) which would not be easy with just a plain text answer.

The goal of this survey is to get a pulse on how effective we are with communicating Fedora CoreOS specific information to the community.

Hi @cverna we discussed this in the Mindshare Committee meeting and we are generally on board with this. We do have some collective survey experience and think a couple of the questions could be improved to get a clearer understanding of sentiments. Based on the Mindshare conversation, I am proposing some changes below. Please let us know what you think

Sorry I let this without an answer for a while.

The suggestions looks good to me except for the
Do you use Fedora CoreOS? - Yes, all the time - Yes, for particular use cases - No, but I have used it in the past - No, I have never used it
And in particular the Yes, all the time answer, since FCOS is targeted at a particular use cases (running containers on servers) it feels weird to have the Yes, all the time choice.

I think the following would be better.
- Yes - No, but I have used it in the past - No, I have never used it
