Includes a new doc about writing policies, to fix #150.

Although I just realised #150 also asked for a doc on how to test a new policy you have written, which is not covered by the doc as is.

I will just add a section showing how to use the dev server and send it an HTTP request asking for a decision, to see your test policy in action.
