#1511 Host OTP from previously added host appears in new host's edit page

If you add/edit a host and set an OTP for the host, then add a new host. When
you edit the new host, the OTP from the previous host is shown for the new
host.

- Add a new host
- Set an OTP for the host
- Add another host
- Edit that host
