#1061 installing dogtag creates a unused naming context in the CA DS instance

When we install dogtag on ipa 2 naming contexts are created in the CA instance.
One is named after the IPA realm and is unused. and one is always named o=ipaca

We should not create (or we should remove) the first one.

Suggest addressing this when Dogtag is refactored. Deferring for now. It is not a 3.0 target.
