The statement from the documentation, "enter your password, followed by the OTP token value" is somewhat misleading for a newcomer. Based on those instructions, I was "entering my password" (i.e. typing my password followed by the "enter" key) and expecting a second prompt for the "OTP token". What is actually required is to "type your password immediately followed by your OTP token and press enter to submit them together as a single concatenated line".
