Looks like the comment at http://trac.edgewall.org/ticket/6032#comment:12 helped out.

There was a ticket that had only custom fields, not the normal ticket ones, so then when it tried to create it it picked the same id and blew up. ;(

I'm really not sure what caused it, but I manually fixed the db for now.

Please let me know if it's not working.
