Make the document explicitly state that the review ticket should not be
closed upon approving.

This should help avoid cases like this:

According to the policy in README, commit access is granted to everybody with a merged pull request. Therefore I have added you to package-maintainer-docs group. You can now directly commit trivial fixes if you prefer that. Pull requests are also accepted for any kind of change, and strongly preferred for anything nontrivial.

(Sorry this happened so late - it is a manual process, so if I happen to be busy when we get a new contributor, I may forget about it.)
