#151 Improve readability of CommOps toolbox links

Download 151.patch

Quick change to the format of the tools in the CommOps toolbox. It makes the titles of the tools as hyperlinks instead of leaving awkward hyperlinks below the titles.

This also renames the index.adoc file to something more obvious, commops.adoc. I updated the _topic_map.yml to reflect this change.
