Description of problem:
CRL Issuing point with empty name is added when Cancel button is clicked on the
window where we add a CRL Issuing point.

How reproducible:
always

Steps to Reproduce:
1. In pkiconsole click CRL Issuing point under Certificate Manager
2. Click the Add Button
3. In the window that opens up, click cancel

Actual results:
Issuing point is created in the left navigation pane with no name

Expected results:
The window that opens up for adding CRL issuing point should be closed when
Cancel button is clicked.
