#464 Instance name should be defined in [Tomcat] or [Apache]

Currently the pki_instance_name has to be defined in [DEFAULT] in the user deployment config because there are other parameters in [DEFAULT] that are interpolated using the instance name.

It would make more sense to define the instance name in [Tomcat] or [Apache] as it would allow different instance names for different application servers. Also, the [DEFAULT] is actually a special section in Python's ConfigParser which doesn't really need to be exposed to the end users.
