scep-submit: URL encode CA identifiers
    
    When asking for a CA's capabilities list, its certificate, or its
    certificate chain, we weren't URL-escaping the values that were passed
    in via the -i flag or the $CERTMONGER_SCEP_CA_IDENTIFIER environment
    variable when constructing the request to send to the server (more of
    ticket #50).
