#12 Both responder_cmd.h and nsssrv.h implement nss_ctx differently

responder_cmd.h has an implementation of nss_ctx that varies from nss_ctx in nsssrv.h.

It is inappropriate for responder_cmd.h to be using an nss_ctx anyway. It should really be a responder_ctx. sss_process_init() should be updated similarly.
