What version of nss and openldap are you using? rpm -q nss openldap

nss-3.15.3-6.el6_5.x86_64
openldap-2.4.23-34.el6_5.1.x86_64

What version of the nss-softokn package is installed? The nss-softokn-3.14.3-10.el6_5 package was recently released, and I'd like to see if you are running with that version.

rpm -q nss-softokn
nss-softokn-3.14.3-10.el6_5.x86_64

It turned out this is a bug in NSS. Closing this ticket as a duplicate of the bug:

If you run into the problem again, please let us know and try the nss patch.
